- DELED Entrance Result, Admit Card
- College Choice Filling Form
- मैट्रिक परीक्षा पास करने का मूल प्रमाण पत्र एवं अंक प्रमाण पत्र की मूल कॉपी
- इंटर परीक्षा का मूल अंक एवं प्रमाण पत्र की मूल कॉपी
- आवासीय प्रमाण पत्र का मूल प्रति
- आरक्षित कोटि एवं विशेष आवश्यकता वाले अभ्यर्थी सक्षम पदाधिकारी से निर्गत जाति एवं विकलांगता प्रमाण पत्र की मूल प्रति
- अंतिम शिक्षण संस्थान से निर्गत महाविद्यालय परित्याग प्रमाण पत्र की मूल कॉपी
- सभी मूल प्रमाण पत्र स्व अभिप्रमाणित छाया प्रति का दो सेट, तीन पासपोर्ट साइज फोटो एवं सभी मूल प्रमाण पत्र एवं अभिलेख दो फाइल फोल्डर में जमा करेंगे
- सत्यापन के क्रम में या उसके बाद प्रमाण पत्रों में फर्जी पाए जाने की स्थिति में नामांकन के दावे को रद्द करते हुए कानूनी कार्रवाई की जाएगी
- अभ्यार्थी द्वारा नामांकन शुल्क के रूप में जमा की गई राशि किसी भी स्थिति में वापस नहीं की जाएगी
शपथ पत्र मूल रूप में अनुमंडल पदाधिकारी/ कार्यपालक दंडाधिकारी द्वारा जारी की गई की मेरे द्वारा दिए गए अंकपत्र एवं सभी प्रमाण पत्र सही है तथा मै अन्य किसी संस्थान में नामांकन नामांकित नहीं हूं |
